Scheduling an Appointment for New Patients:
HKSI is currently only accepting new patients with certain conditions.
Please have your referring provider send all medical records and an appointment
request to (717) 526-4476. Dr. Diamond will review the records and
determine whether he will take the case on.
What to Expect at Your First Appointment:
As a new patient to our practice you should receive a welcome packet about a
week prior to your scheduled appointment. This packet will include a brief
history form, medication list form, directions, HIPAA form, and a copy of our
financial policy. Please fill the forms out in their entirety and bring them to
your appointment.
Please arrive 15 minutes early to fill out forms prior to your appointment.
Your first appointment will last approximately 15-45 minutes. Your
provider will discuss in detail why you are seeing a nephrologist and discuss a plan of treatment. You do not need to fast for your appointment or give a urine specimen. Laboratory studies will typically be ordered for follow-up visits and a lab order script will be given at the time of checkout. You may take the lab order script to a lab of your convenience. Any other diagnostic testing needed will also be scheduled at the time of checkout.
We understand that seeing a new provider at times can be overwhelming, please do not hesitate to contact our office if you have any questions regarding your visit.
For Follow-Up Appointments:
Our office staff will issue reminder calls when time permits-please make sure we have your current contact information on file. If you need to reschedule your appointment, please kindly give us at least 24 hours' notice. A fee will be charged for no-showing appointments.
Please be sure that you have completed all ordered testing prior to your appointment. If not asked otherwise, laboratory studies need to be drawn 1-2 weeks prior to your follow-up visit to ensure processing time for results. If you have lost your laboratory order script you may contact our office for a new copy. Please be aware that your appointment may be rescheduled if all testing is not completed prior to your follow-up appointment.
